Patients have the option of making an appointment through our website, by email, or by calling in. All scheduling of office visits is done by our appointment scheduler (Tara) at 850-916-8680 or the reception desk. Except in the case of emergencies, it is not our policy to see patients without an appointment. Our doctors fill their schedules well in advance with, in some cases, people who have waited weeks to make an appointment.
Dr. Nilssen’s Clinical Schedule: Every Tuesday and Thursday
Dr. Murawski’s Clinical Schedule: Every Monday, Wednesday and Friday (half-day)
Please bring the following items with you to your appointment:
Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary care physician authorization before treatment is given. Please check with our insurance if you are in doubt.
X-rays taken in the past 3 months of your appointment date and MRI if you have had one. You will need to bring the actual disc drive or the hard copies; pictures or print outs of the images will not be accepted. Image reports are also helpful if available.
Clinic notes and/or operative reports from your referring physician or any previous treatment notes relating to your condition.
Payments are due in full at the time of your office visit. Please check with your insurance to make sure that Dr. Erik Nilssen or Dr. Daniel Murawski are listed as “in-network” providers under your policy prior to your visit.
We make every effort to process all medical records requests within 2 business days. In the event that you would like our office to release your records to another physician or vice versa, you will need to fill out an Authorization to Release Medical Information Form (link to resource?)
